Important: The Password policy has changed, effective July 15, 2011. The next time you change your password, it should meet the following criteria:
  • Should not contain user id in it
  • Should be at least 8 characters
  • Should not be the same as user id
  • Should contain at least a number, an alphabet (lower case) and a special character.
  • Last 10 previous passwords cannot be reused
  • Should not reuse the password that was set in the last 90 days
  • Should reset password at least once per 90 days

The EPSDT data entry application is no longer available. As stipulated by AHCCCS, providers should use EPSDT tracking forms or equivalent Electronic Medical Record process to document all age-specific information related to EPSDT screenings and visits. Following each EPSDT visit, a copy of the EPSDT information must be maintained in the member's chart, and the same information must be sent to UnitedHealthcare Quality Management Attn: EPSDT, 1 East Washington, Suite 900, Phoenix, AZ 85004.
